Notice of destruction of Special Education records

Hermiston School District wants to inform parents/guardians and former Special Education students that on October 1, 2018, the district will destroy the records of students who ended/exited Special Education Services from 1980-1998.

 

Special Education records include identification, evaluation, educational placement, or the provision of occupational therapy, physical therapy, speech-language pathology or other related services made available as determined through the Individual Education Plan process. 

 

These records are no longer needed by the district but may be useful to the parent/guardian or former student who may apply for social security benefits, rehabilitation services, college entrance, etc.

 

If a student wants to obtain their record, he/she needs to sign a consent for release of information and provide photo identification. The record can be obtained at the Hermiston School District Office in person or can be faxed. All unclaimed records will be destroyed on October 1.
